The pitching log form to be kept by all managers in the Allen Baseball Association. The form is to be exchanged prior to the beginning of each game during the managers meeting.

Medical Treatment Consent Form

Medical Treatment Consent Form. Managers should have parents sign these. Managers should maintain the signed forms for their use during the course of the season.

Allen Sports Association Sponsorship Form

The administrative fee is 20% per sponsorship with a $100 cap per sponsorship. If you have never used the ASA sponsorship program please contact your Sports Specialist for information regarding requirements and expectations. Old versions of the ASA sponsorship form will not be accepted.

Playing Up Consent Form

This form should be signed by parents and submitted for any player electing the play-up option.

Host Family Program Description

A description of the Host Family program in place when Allen Baseball hosts a Babe Ruth tournament.

Player Release Form

Managers of 9U teams and above should use this form to release players at the end of the spring season. All player release forms are due by July 1. Forms should be faxed to the ASA office at (972) 727-9760.
